Effective May 23, 2008, federal regulations require:
American Republic Insurance Company does not require the NPI on paper claims, although this information is helpful in the processing of the claim. Some provider networks do require the NPI on paper submissions and will reject claims which do not contain this information. We recommend that all providers review the NPI requirements of the networks in which they participate. Paper claims should be submitted on the most recently revised claim forms (CMS 1500 version 02/2012 and UB-04).

When you call in, you will be prompted to enter your patient’s policy number and date of birth before you can access policy-specific details.
You may view patients' claim submission information by selecting the appropriate PPO network below. To process claims more efficiently, please submit claims electronically using the EDI Payer ID. Please note: Claims from Medicare are considered crossover claims, so submitting a paper claim results in unnecessary duplicates.
