Pre-existing conditions

This coverage is designed to pay for accidents that occur or sickness that first manifests itself after the date of issue. We will not pay for a pre-existing condition or disease for up to 2 years after issue that is not admitted on the application (6 months if it would not have affected the issuance of the coverage). Pre-existing condition means the existence of symptoms that would cause an ordinarily prudent person to seek diagnosis, care, or treatment, or a health condition for which medical advice or treatment was recommended or received within a 6-month period prior to issue. Pre-existing conditions admitted on the application will be covered after 6 months following the issue date, unless excluded by name or specific description. Any false statement, misrepresentation, or omissions in the application may result in benefits being denied or the contract being rescinded, subject to the Time Limit on Certain Defenses. (Provisions may vary by state.)

You may renew this coverage for life by paying the premiums as they come due. A 31-day grace period is allowed for payment of your premium. All applications are underwritten individually, and each person is assigned a rate class. We reserve the right to change the premium rates on any renewal date but only if the change is made for all policies in the same class and not on an individual basis. The total premium you pay each year for your coverage may vary depending on the mode (frequency of payments) and method you select for premium payment. Your premium may change if you change your benefits. Your benefits and premiums will vary depending on the plan, coverage choices, and each optional benefit selected.

Arizona Exclusions and Limitations

This coverage does not cover loss that results from:

  • Any confinement or care due to an injury or sickness that occurs prior to the effective date of the coverage
  • Treatments or services not specifically provided in the coverage and any complications
  • Alcohol or drug abuse or any complications
  • Confinements or care due to psychiatric, mental, or nervous conditions without demonstrable organic disease except those caused by conditions that are physical in nature, such as Parkinson’s disease, Alzheimer’s disease, or similar forms of irreversible dementia
  • Suicide, attempted suicide, or intentionally self-inflicted injury
  • Confinement or care that is covered by any state or federal worker’s compensation, employer’s liability, or occupational disease law
  • Any services or treatments received outside the United States or its possessions
  • Cosmetic surgery or other elective procedures that are not medically necessary or any complications
  • War
  • Committing or attempting to commit a felony or engaging in an illegal occupation
  • Intoxication or being under the influence of any narcotic, unless taken on advice of a physician
