Preauthorization Program

The Benefits of Preauthorization

An important part of many of our health insurance programs is preauthorization. Preauthorization helps to ensure member benefits are used efficiently and cost effectively. We work in partnership with you to ensure our members get the appropriate care in the appropriate setting for their medical condition.

American Republic Insurance Company uses Utilization Review Accreditation Commission (URAC)-accredited utilization management companies dedicated to serving as a health care advocate for our members.

Depending on the Type of Coverage, Preauthorization May Be Required for:

  • Inpatient Hospitalizations
  • Certain Surgeries
  • Home Health Care
  • Home Infusion Therapy
  • Hospice Care
  • Outpatient Physical, Occupational and Speech Therapy
  • Certain Prescription Medications

Preauthorization is not required for regular doctor's office visits.

Preauthorization is not a guarantee of benefits. Payment of benefits is subject to the terms and conditions set forth within the policy/certificate, including out of network provisions. The member is responsible for any deductible, coinsurance or copayment described in the policy/certificate.
