Remember, preauthorization may be required for inpatient
hospitalizations and some surgeries, but not for regular doctor's office
visits. Depending on your coverage, there may be other medical services and
treatments that require preauthorization, such as home health care, home
infusion therapy, hospice care, outpatient therapies or certain prescription
drugs.
If your coverage requires preauthorization, the list of medical
services and treatments requiring preauthorization will be listed on the back
of your identification card. Please remember that preauthorization is
ultimately your responsibility. Benefits may be reduced for failing to obtain
preauthorization when required.
